BBL Recovery: What Dr. Salama’s Patients Experience

A Brazilian butt lift is two operations in one: liposuction of the abdomen, waist and back, and fat transfer to the buttocks. Recovery is therefore about two things at once: letting the liposuctioned areas settle, and protecting the transferred fat while it gains a blood supply. The rules that follow, no sitting on the buttocks for six weeks, the garment, the pillow, the massages, exist to protect the result you paid for. This guide sets out the plan board-certified plastic surgeon Dr. Moises Salama gives his BBL patients at Elite Plastic Surgery in Hollywood, serving Miami, week by week.

The First Few Days

You go home the same day in your compression garment with foam pads over the liposuctioned areas, and you need an adult with you for the first few days: a friend, family member or a care professional. The buttocks feel full and tight but are usually not very painful. The abdomen, flanks and back, where the fat was harvested, are sore, bruised and swollen, and fluid tinged with blood leaks from the small incisions for the first 24–48 hours; that is expected, and absorbent pads under the garment manage it.

  • Walk. Short walks every couple of hours from the first evening, and at least ten minutes a day thereafter, keep the blood moving in the legs and lower the risk of clots. Stretch your legs when resting.
  • Stay off the buttocks. Stand, walk, kneel or lie on your stomach. When you must sit, use the BBL pillow under the thighs, and keep it short.
  • Medication. Take the pain medication and antibiotics exactly as prescribed. Fill the prescriptions before surgery.
  • Drink water and eat a high-protein diet. Swelling and fluid shifts make you feel bloated and heavier on the scale for the first week; that is fluid, not fat.
  • Shower from 48 hours with the garment off, pat the incisions dry and put the garment and foams straight back on.

Sitting and Sleeping: The Six-Week Rule

Transferred fat arrives in the buttock as thousands of small parcels with no blood supply. Over the first weeks, new capillaries grow into them; until they do, the cells live by diffusion and die if they are compressed. Sitting on the grafts in the first weeks crushes them and you lose volume exactly where you sat. That is why Dr. Salama asks for six weeks without direct pressure.

  • Sitting: for the first six weeks, only short periods, on a BBL pillow placed under the thighs so the buttocks hang free behind it. Stand and walk every 20–30 minutes. In a car, the pillow goes under the thighs with the seat reclined slightly; drive only once you are off prescription medication.
  • Toilet: fine; the pressure is on the thighs.
  • Sleeping: on your stomach for six weeks, with a pillow under the hips if the lower back aches. Side sleeping is acceptable with a pillow behind you so you do not roll back. Lying on your back starts at six weeks.
  • Week six onward: sit normally on soft surfaces; by week eight on any surface.

The Compression Garment (Faja), Foams and Board

The garment compresses the liposuctioned areas, not the buttocks. It limits swelling and fluid collection, helps the loosened skin retract to the new contour and supports the lower back. Dr. Salama calls the garment and the lymphatic massages the two crucial components of a good post-operative result, alongside good technique in the operating room: both maintain the shape created in surgery and keep lymphatic fluid draining, so inflammation stays even and the areas heal smooth, without irregularities. His schedule:

Weeks What you wear
0–2 Full garment at all times except to shower, with foam pads over the abdomen, flanks and back.
2–8 Full garment; the front foam is replaced by an abdominal board (flattens the abdomen and prevents creasing); side and back foams stay until week 6–8. If the garment loosens as swelling falls, buy a smaller size or have it taken in: it must stay snug.
8–12 Waist trainer with a T-shirt and the board, no foams, in place of the full garment. If you want more compression elsewhere, wear the waist trainer over the full garment with the board only.
12–24 Compression is optional, but Dr. Salama recommends a garment or waist trainer by day or at night for another three months for the best contour.

Wear loose, breathable bottoms over the garment in the first weeks; no jeans. Moisturize the skin of the lipo areas daily once the incisions are closed, because compression dries it.

Lymphatic Massage and Vacuum Therapy

Liposuction leaves fluid and small pockets of tissue injury under the skin; without treatment they can harden into lumps and uneven areas. Lymphatic drainage massage moves the fluid toward the lymph nodes, speeds the fall in swelling and keeps the skin smooth. Dr. Salama sets the number and timing of sessions at your first follow-up, and most patients start in the first week. Before each session, apply an electric heating pad over a towel to each lipo area for ten minutes to soften the tissue. The buttocks are never massaged, cupped or compressed during recovery. Vacuum (cupping) therapy, when Dr. Salama recommends it, is used on the liposuctioned areas later in recovery to smooth contour; it is not applied to the grafts.

BBL Recovery Week by Week

Time after surgery How you feel What you can do What to avoid
Days 1–3 Swollen, bruised and sore in the lipo areas; buttocks tight and full; incisions leaking fluid Walk every 2 hours; shower at 48 hours; caregiver at home Sitting on the buttocks, lying on your back, lifting
Days 4–7 Bruising at its most colorful; swelling at its peak; off prescription pain medication for most Lymphatic massages begin; longer walks; drive once off narcotics and comfortable on the pillow Heat on the buttocks, baths, pools, alcohol
Week 2 Soreness easing; bruising fading; buttocks still firm and swollen Return to desk work with the pillow; front foam swapped for the board Long sits without standing breaks, exercise
Weeks 3–4 Comfortable walking; skin of the lipo areas numb or tingly; fluffing begins Light exercise (stationary bike, incline walking) at 4 weeks; sexual activity at 4 weeks; garment taken in if loose Squats, running, sitting unsupported, dieting
Week 6 Swelling down by half or more; buttocks softening Sit normally on soft surfaces; lie on your back; regular exercise, swimming and the beach with scars covered; side and back foams come off by week 8 Tanning the scars
Weeks 8–12 Feel normal; shape close to final Waist trainer replaces the full garment; glute training; sit on any surface Rapid weight loss
Months 3–6 Final softness and shape Compression optional (recommended); photos for your 3- and 6-month comparison

Swelling, Fat Survival and “Fluffing”

Your buttocks will look largest in the first week, when swelling adds to the transferred volume, then shrink over weeks two to eight as the swelling leaves and the fat that did not gain a blood supply is absorbed. Then the surviving fat softens, the skin relaxes and the shape rounds out; patients call this fluffing. In Dr. Salama’s hands, with gentle power- and ultrasound-assisted harvesting, placement above the muscle under ultrasound guidance and six weeks of protection, 60 to 80 percent of the transferred fat survives permanently. The result at three months is close to final; the last changes finish by six months. What lowers survival: sitting on the grafts, smoking or nicotine, crash dieting in the first three months, and pressure from tight clothing over the buttocks.

Diet, Weight, Alcohol and Smoking

  • Keep your weight stable for three months. The grafted fat behaves like fat anywhere else: lose weight and it shrinks, gain weight and it grows. Dieting in the first months, while the fat is establishing itself, costs volume you cannot get back.
  • Eat protein and healthy fats, plenty of fruit and vegetables, and drink water through the day; both healing and fat survival depend on it.
  • No smoking, vaping or nicotine for a month before and a month after surgery. Nicotine narrows the capillaries that the grafts depend on.
  • No alcohol for the same period; it dehydrates, increases swelling and does not mix with pain medication.
  • No herbal supplements or blood thinners (fish oil, vitamin E, ginkgo, garlic, aspirin, ibuprofen) for two weeks before and after, unless Dr. Salama says otherwise.

Incisions and Scars

A BBL leaves several incisions of a few millimeters: at the liposuction access points on the abdomen, flanks and back, and one or two on each buttock where the fat was placed. They drain fluid for a day or two, close within about a week, and fade to small pale marks over 6–12 months. Keep them clean and dry, change the absorbent pads as needed, and start silicone gel or tape once they are fully closed. Keep them out of the sun for a year; at the beach from week six, cover them.

Normal vs. Call the Office

Normal in the first weeks Call us the same day
Blood-tinged fluid from the incisions for 24–48 hours Fever over 101°F or chills
Bruising that drifts toward the thighs and groin Spreading redness, warmth, pus or an incision that opens
Firm, lumpy or numb areas in the lipo zones that soften with massage over months One area much more swollen, hard and painful than the rest
Buttocks that look too big, then shrink: swelling, then fluffing Calf pain, swelling of one leg, chest pain or shortness of breath
Itching as the skin heals; tiredness and low mood in week one Rash or hives from a medication

Recovery When a BBL Is Combined With Other Procedures

  • BBL with a tummy tuck: the tummy tuck sets the pace: a bent-at-the-waist posture for the first week, drains for about a week, and you must avoid both sitting on the buttocks and lying flat on your back, so recovery is spent on your side or stomach with pillows. Plan 2–3 weeks off work. Dr. Salama discusses whether to combine or stage the two at your consultation.
  • BBL with J-Plasma/Renuvion skin tightening: more swelling and firmness in the treated skin for the first weeks; the garment schedule is the same.
  • Skinny BBL (see the page): identical rules; because less fat is transferred, protecting every graft matters even more.

BBL Recovery Frequently Asked Questions

How long is BBL recovery?

Plan on about two weeks off work, six weeks before you sit normally or exercise fully, eight weeks in the compression garment, and three to six months for the swelling to clear and the final shape to appear. Most patients say they feel completely normal at about two months.

How long after a BBL can you sit?

Dr. Salama asks patients not to sit directly on the buttocks for six weeks. From day one you can sit for short periods on a BBL pillow placed under the thighs, which keeps the grafted fat off the seat. At six weeks you can sit normally on soft surfaces and start lying on your back.

How do you sleep after a BBL?

On your stomach for the first six weeks; on your side is acceptable if you place a pillow so you do not roll onto your back. Lying on your back is allowed from week six. A pregnancy pillow or a wedge under the chest makes stomach sleeping easier.

How painful is BBL recovery?

The buttocks themselves are usually only mildly sore. The liposuctioned areas, the abdomen, flanks and back, feel like a hard workout with bruising and tightness for the first one to two weeks, and the skin is tender and sometimes numb for longer. Prescription pain medication covers the first days; most patients are on over-the-counter medication within a week.

How long do you wear the faja after a BBL?

The full compression garment is worn at all times, except to shower, for eight weeks. The front foam is replaced with an abdominal board at two weeks; the side and back foams stay for six to eight weeks. From week eight a waist trainer with the board can replace the full garment, and Dr. Salama recommends compression by day or at night for another three months for the best contour.

When can I drive after a BBL?

Once you are off prescription pain medication and can sit on your BBL pillow comfortably, usually at one to two weeks. Keep drives short in the first weeks and stop to stand and walk every 20–30 minutes.

When can I go back to work after a BBL?

Plan about two weeks off. Desk work resumes at two weeks with the BBL pillow and regular standing breaks; physical jobs wait until Dr. Salama clears you, usually four to six weeks.

When can I exercise after a BBL?

Walking starts the day of surgery and continues daily. Light exercise such as a stationary bike or incline walking begins at four weeks; regular exercise, including swimming and the beach (with the scars covered), at six weeks. Squats and glute training, done without sitting, help the result once you are cleared.

What are the massages after a BBL?

Lymphatic drainage massage of the liposuctioned areas moves retained fluid out, lowers swelling and keeps the skin smooth. The schedule and number of sessions are set by Dr. Salama at your first follow-up. Before each session, a heating pad on the lipo areas for ten minutes per area (with a towel underneath) loosens the tissue. The buttocks are not massaged.

What is BBL fluffing?

Right after surgery the buttocks are swollen and firm. Over weeks two to twelve the swelling clears, some of the transferred fat that did not gain a blood supply is absorbed, and the surviving fat softens and settles, which patients call fluffing. The result at three months is close to final; the last changes happen by six months.

How much fat survives a BBL?

In Dr. Salama’s experience, 60 to 80 percent of the transferred fat survives permanently when the fat is harvested gently, placed above the muscle under ultrasound guidance and protected from pressure during the first six weeks. Smoking, sitting on the grafts and rapid weight loss lower survival.

Can I lose weight or diet after a BBL?

Not in the first three months. The grafted fat needs calories and a stable weight while it establishes its blood supply; crash dieting at this stage shrinks the result. Eat a high-protein, moderate-calorie diet, and leave weight loss until the fat has fully settled.

When can I shower, swim or take a bath after a BBL?

Shower from 48 hours, with the garment off and the incisions patted dry afterward. No baths, pools, hot tubs, saunas or the ocean for at least four weeks; swimming and the beach are usually fine at six weeks with the scars covered.

When can I have sex after a BBL?

At about four weeks, in positions that keep pressure off the buttocks. No restrictions once Dr. Salama clears you at six weeks.

What are the warning signs after a BBL?

Call the office the same day for fever over 101°F, spreading redness or discharge at an incision, one area becoming much more swollen, hard or painful than the rest, calf pain or swelling of one leg, chest pain or shortness of breath. The after-hours number is on your discharge instructions.

Can I fly home after a BBL?

Out-of-town patients stay in the Miami area for about 10–14 days for the early follow-ups and massages. For the flight, bring your BBL pillow, wear the garment, walk the aisle every hour and keep your legs moving; longer flights are easier after week two.
